Do I need a police complaint?
For a lost or stolen licence, RTOs commonly ask for one. For a damaged licence, bring the damaged card.

Do I need to come to the office?

An affidavit is stamped under Article 4 of the Karnataka Stamp Act — ₹100 e-stamp paper. Bonds such as an indemnity bond use their own article.
